Provider Online Services registration application form

This form can be used by health services to request access the Provider Online Services (POS). This is a secure internet portal owned and operated by the Transport Accident Commission (TAC) and WorkSafe Victoria. POS enables you to check a patient's TAC / WorkSafe Victoria claim status.
