About the TAC’s rehabilitation at home program

We can help pay for our eligible clients to undertake a rehabilitation at home program. This allows them to return home from hospital sooner by receiving home-based support rather than rehabilitation in an inpatient hospital setting.

Rehabilitation at home is generally delivered over a period of up to 8 weeks, depending on the client’s needs. The program can be offered to our clients who are clinically independent and medically stable.

Referrals to a rehabilitation at home program can come from surgeons or hospital staff.

Becoming a TAC rehabilitation at home provider

If your organisation would like to deliver rehabilitation at home to TAC clients, you will need to register with us and agree to our provider guidelines and service standards.

To apply, please contact us at sectors@tac.vic.gov.au to request an application form and fee schedule.

Once registered, you can deliver services through your team of professionals. Depending on our client’s needs, this can include a physiotherapist, occupational therapist, nurse, social worker, rehabilitation physician and rehabilitation at home case coordinator.

By becoming a TAC rehabilitation at home provider, you will:

  • Expand the rehabilitation options and choice for Victorians injured in transport accidents
  • Give TAC patients increased access to home-based care
  • Increase service access for patients in rural and regional areas
  • Receive feedback about your performance as a rehabilitation at home provider
  • Contribute to the long-term evaluation of rehabilitation at home versus traditional inpatient care, helping to build the evidence base and research

Provider requirements

We can pay for a client’s rehabilitation at home services for a period of 8 weeks, up to the value of $3,000.

In the first 90 days after the client’s accident, we can help pay for rehabilitation at home services without the need for the client, their hospital discharging team or their rehabilitation at home provider to contact us for approval first. We ask you submit our Rehabilitation at home initial assessment form to ensure outcomes and provider monitoring can be performed. However, you can begin delivering services without waiting for approval from the TAC.

If services are to begin more than 90 days after the client’s transport accident, you will need to request this by completing our Rehabilitation at home initial assessment form. If services are approved, we will confirm this by letter or email. For a more timely answer, please phone us.

If you believe the client will need services beyond 8 weeks or $3,000, submit a Rehabilitation at home extension request form for review. Please provide this as soon as you anticipate further services will be required. We need this request this no later than 2 weeks before the end of the approval period to avoid disruption to the client’s services.

At the conclusion of services, please complete our Rehabilitation at home discharge form.

You will also need to send us monthly updates on key performance indicators.
